Working within CCP’s community based services, you will be working from Gloucester Royal Hospital (GRH).

Housing related problems can cause hospital admissions, prevent discharge, and delay the recovery of patients. Multi-agency working is vital within this role, as you will be working alongside hospital staff, social care and other teams based within Gloucester Royal Hospital.

You will be proactive in providing housing advice, assistance and support to patients. You will also play a part in the patient discharge process by addressing any delays caused by housing issues.

Furthermore you will be supporting the wider hospital team by offering guidance and training to the staff. Generally working hours will be on a 09:00-17:00 basis, but flexibility is required for evening and weekend work to meet the needs of the patients.
