Our team based in Cheltenham, is expanding into new and exciting areas and we have a new opportunity that might be suited to you.

The role of a Social Prescribing Link Worker is to provide a diverse range of interventions to adults who are referred to the service through GP Practices. The post holder will empower individuals to take control of their health and wellbeing through referral to non-medical ‘link workers’ who give time, focus on ‘what matters to me’ and take a holistic approach, connecting people to community groups and statutory services for practical and emotional support. You will work as part of the Cheltenham Central Primary Care Network.

Duties include:                                                                                                                                         

Taking referrals from health care professionals and staff within Primary Care Networks (PCNs) and referrals from a wide range of agencies, working with GP practices, pharmacies, multi-disciplinary teams, hospital discharge teams, allied health professionals, fire service, police, job centres, social care services, housing associations, and voluntary, community and social enterprise (VCSE) organisations.

Working within communities, to establish needs based on a whole family assessment and contribute to forming a robust action plan agreed with the family and other key agencies.

Assisting individuals to take decisions on what type of support they require that best meets their health and wellbeing needs. This will involve using a range of behavioural change techniques such as; motivational interviewing, health coaching and strengths-based approaches to determine their skills, interests and motivation.

Co-producing and implementing strength-based Personal Development Plans & Family Plans based on the their priorities, interests, values and motivations – including what they can expect from the groups, activities and services they are being connected to and what the they can do for themselves to improve their health and wellbeing.

Providing a range of interventions that support the agreed outcomes such as parenting skills, behavioural, anger management and social isolation.
