Community Wellbeing Social Prescriber

Full-Time – 37.5 hours per week (job share considered)

Salary -  £17,000 - £19,000 per annum (+ benefits) – pro rata for part-time

The Community Wellbeing Service works to improve general wellbeing and meet people's wider social needs. You will assist people to make informed choices by providing relevant information and connecting people to local services, organisations and groups.

As a Social Prescriber you will be providing people with non-medical prescriptions empowering people to take control of their own health and wellbeing.
